Immunizations

New York State requires the following immunizations for school entry or attendance. (See the NYS website for complete details)

 

Diphtheria, Tetanus, Pertussis (DTaP or DTP)
3 doses
Polio (OPV or IPV)
3 OPV or 4 IPV
Measles Mumps, Rubella (MMR)
2 doses
Hepatitis B (HepB)
3 doses
Haemophilus influenzae type b (Hib)
3 doses for preschool or day care
Varicella
Born on or after 1/1/98 1 dose

The American Academy of Pediatrics recommendations for some immunizations are higher than New York State's recommendations. To see their recommendations visit their site. You can discuss the differences with your healthcare provider. The CDC also offers a good reference for updated Child & Adolescent Immunization Schedules.

For your convenience we have placed pdf versions of the Vaccine Information Statements (VISs) on this website for you to read. Vaccine Information Statements (VISs) are information sheets produced by the Centers for Disease Control and Prevention (CDC) that explain to vaccine recipients, their parents, or their legal representatives both the benefits and risks of a vaccine. Federal law requires that VISs be handed out whenever certain vaccinations are given. A VIS with a date several years old is not necessarily out of date. VISs are not updated annually or on any other routine schedule. An updated VIS with a new date is posted when there is a change in recommendations.
