Information
We
are considered "Participating Providers" for all
healthcare plans listed below. As a Participating Provider,
we will submit all claims on behalf of the patient/responsible
party. All insurance companies that require the patient
to pay a co-pay, require this payment to be made at the time
of service.
Healthcare and insurance rules continue to change and we strive to provide quality care for all ages. Services are defined by the government services programs and commercial health insurers. A physical exam and follow-up services are defined here.
If
the co-pay is not paid at the time of service, a $10 administrative
fee is added to the patient's bill.
If
you have a health care plan which is not listed below, we may still
accept payment from them. Our patient accounting department
will assist you in filing these claims. In some cases the
patient is required to pay in full at the time of service
and then submit the bill to the insurance company for reimbursement.
Patients
who do not have insurance are required to pay in full at
the time of service.
For
patients who are required to pay in full at the time of
service, a $10 administrative fee is added if we are required
to bill you.
