Please
arrive at your doctor's office 20 to 30 minutes prior to your
first scheduled appointment. Information will be requested
for insurance
and financial purposes. Please bring any insurance cards or
papers with you on your first visit. After this initial visit,
you will only need to check-in at the front desk for your appointments.
You will need your insurance card for each appointment.
If any change in your name, address, phone number, marital
status,
employment,
or insurance carrier occur, please notify us promptly.
Please
note that we will also need the information of the insurance
subscriber as well as that of the patient.
Wheelchairs are available for patient use while
at the Canandaigua Medical Group. They may be obtained at the main entrance.

Referral Requirements
Insurance Information
It is extremely important for you to know the requirements of
your insurance coverage for any office visits and/or procedures.
This is particularly true if you are in a managed care plan.
If you have any questions regarding coverage, you can contact
your insurance patient representative for assistance.
Referral Authorization Expectations
If your health care insurance requires a referral from your
primary care physician, you are required to obtain this referral
prior to arriving for a specialty physician office visit. Please
contact your primary care physician office prior to arriving
for your appointment to obtain the necessary referrals.
