Canandaigua Medical Group introduces new appointment confirmation and outreach system for our patients.
We are pleased to announce a new proactive patient appointment confirmation and recall system that will help us better manage your care and help organize your busy life.
The friendly appointment confirmation system will remind you of the date and time of an upcoming appointment. The interactive system allows you to confirm the appointment or to cancel or reschedule the appointment if you need. Everything is done through your touch tone phone and there are even safety features allowing you to go back if you press the incorrect response.
The recall system is an innovative method of improving patient quality and safety. You can expect our system to contact you to:

Dr. Hani Chehata joins the Canandaigua Medical Group in the Ob/Gyn Department
The Ob/Gyn Department at the Canandaigua Medical Group is pleased to announce the arrival of their new full-time Ob/Gyn Physician, Dr. Hani Chehata. Dr. Chehata will begin seeing patients the first week of October.
Dr. Chehata was born and raised in Alexandria, Egypt. He graduated from the Alexandria School of Medicine in 1985 and then went on to complete his internship and residency in Karmouz Hospital, Alexandria, Egypt in 1991. He did his postgraduate training in Obstetrics and Gynecology and became Board Certified in Great Britain from 1991 through 1996. Dr. Chehata came to the United States in 1997 and completed his second residency training in Obstetrics and Gynecology at the George Washington University Medical Center, Washington, DC in 2002. He is Board Certified, a member of the Royal College of Obstetricians in Great Britain and a member of the American College of Obstetricians and Gynecologists. His subspecialty interests include urinary incontinence and infertility.
Dr. Chehata and his wife, Mary (a pharmacist at Rite Aid), currently live in Horseheads, NY with their son, Anthony. They look forward to moving to the Canandaigua area soon. They are also proud members of the Coptic Orthodox Church of Egypt. He enjoys watching and playing soccer, tennis, table tennis, and swimming.
Dr. Mitchko has resumed the responsibilities of Lab Director.
Dr. Michael Mitchko has completed COLA's on-line Lab University Laboratory Director Program to become Canandaigua Medical Group's new Lab Director. This will become effective July 2007. Congratulations Dr. Mitchko!
Welcome to Medicare
Look for your "Welcome to Medicare" letter in the mail. If you have received this letter, please call your doctor to schedule an appointment for your physical. You must take advantage of this offer within 6 months of being eligible for medicare, or it will no longer be available. You become eligible on the first day of the month you turn 65 years of age.
Medical Records
The Canandaigua Medical Group utilizes the services of Smart Document Solutions to copy medical records. This is a national service that is used widely throughout our region.
If you wish to have a copy of your medical records all requests will be processed and billed by Smart Document Solutions.

PATIENT SATISFACTION SURVEY |
In an effort to improve the delivery of our healthcare
services, in June of 1999 we began a biweekly survey of patients
visiting our facility. The survey covers ten key patient satisfaction
issues regarding the appointment, reception, and examination process.
The results of this survey provides us with vital patient input
enabling us to enhance the quality of our patient encounters.
We sincerely appreciate the input from the survey
respondents.
