Canandaigua Medical Group begins using the Rochester RHIO
Starting in August, the Canandaigua Medical Group (CMG) will begin using the Rochester Regional Health Information Organization (RHIO) system to access patient health information electronically. The RHIO currently has laboratory results, imaging results, and medication history data available. This is information that CMG currently has to wait for via mail or fax to receive. By using the RHIO, CMG will be able to access this information faster and more efficiently.
In order for us to receive a patient’s information through the RHIO, we will need the patient to sign a RHIO Patient consent form. The forms are available at our offices, or you can download a copy and mail it to us or bring it to your next visit. The form does allow you to say no to this consent. Please understand that saying no does not remove your information from the RHIO system. We also will still have access to your information in a non-electronic format.

Canandaigua Medical Group introduces new appointment confirmation and outreach system for our patients.
We are pleased to announce a new proactive patient appointment confirmation and recall system that will help us better manage your care and help organize your busy life.
The friendly appointment confirmation system will remind you of the date and time of an upcoming appointment. The interactive system allows you to confirm the appointment or to cancel or reschedule the appointment if you need. Everything is done through your touch tone phone and there are even safety features allowing you to go back if you press the incorrect response.
The recall system is an innovative method of improving patient quality and safety. You can expect our system to contact you to:
- Schedule Preventive Care Appointments
Help manage your chronic conditions by reminding you that you are due for a follow up appointment.
This system is discreet and will only ask that you contact us to schedule an appointment without going into the details of your condition.
This is just another way in which we remain committed to providing quality patient care for all ages.
Welcome to Medicare
Look for your "Welcome to Medicare" letter in the mail. If you have received this letter, please call your doctor to schedule an appointment for your physical. You must take advantage of this offer within 6 months of being eligible for medicare, or it will no longer be available. You become eligible on the first day of the month you turn 65 years of age.
Medical Records
The Canandaigua Medical Group utilizes the services of Smart Document Solutions to copy medical records. This is a national service that is used widely throughout our region.
If you wish to have a copy of your medical records all requests will be processed and billed by Smart Document Solutions.

In an effort to improve the delivery of our healthcare
services, in June of 1999 we began a biweekly survey of patients
visiting our facility. The survey covers ten key patient satisfaction
issues regarding the appointment, reception, and examination process.
The results of this survey provides us with vital patient input
enabling us to enhance the quality of our patient encounters.
We sincerely appreciate the input from the survey
respondents.
