Welcome Jessica L. Schock, Certified Nurse Midwife

Speling Bee WinnersJessica was born and raised in Rochester, New York. She earned her Bachelor of Science in Nursing from St. John Fisher College, and gained experience working at Strong Memorial Hospital as a Registered Nurse in Acute Medicine before specializing in Women's Health.

At Unity Hospital in Rochester, Jessica worked as a labor and delivery nurse at the Family Birth Place. While at Unity, Jessica decided to pursue her Masters in Nursing with a specialty in Midwifery. Her clinical training was at United Memorial Medical Center in Batavia; and in September of 2012, Jessica graduated from Frontier Nursing University ("The Birthplace of Midwifery in America").

Jessica joined the Canandaigua Medical Group in August 2013 and is practicing full scope midwifery and well woman gynecologic care. Jessica is an active member of the American College of Nurse Midwives as well as the New York State Association of Licensed Midwives.

Jessica lives in Canandaigua with her husband, Aaron (Program Manager for Catholic Family Center), and two children, Madison and Noah. They are active members at St. Mary's Church, where her daughter will be starting school this fall. Jessica spends her spare time playing piano, reading, and spending time with family.

Canandaigua Medical Group New Secure Practice Portal Website

Practice Portal LoginThe Canandaigua Medical Group is introducing a new Secure Practice Portal Website that interacts directly with our new Electronic Medical Records System.

Each patient will have their own individual login and password to this Secure site. Patients will be able to request prescription refills, make appointment and referral requests, access their billing summary and pay their bill on line, and access portions of their Electronic Health Record.

The site is available for our Internal Medicine and OB/GYN Patients only at this time.

To receive your secure enrollment information, please contact our office at (585) 393-2811 for Internal Medicine and (585) 393-2800 for OB/GYN. You can also request the information at your next visit.

Canandaigua Medical Group begins using the Rochester RHIO

Starting in August, the Canandaigua Medical Group (CMG) will begin using the Rochester Regional Health Information Organization (RHIO) system to access patient health information electronically. The RHIO currently has laboratory results, imaging results, and medication history data available. This is information that CMG currently has to wait for via mail or fax to receive. By using the RHIO, CMG will be able to access this information faster and more efficiently.

In order for us to receive a patient’s information through the RHIO, we will need the patient to sign a RHIO Patient consent form. The forms are available at our offices, or you can download a copy and mail it to us or bring it to your next visit. The form does allow you to say no to this consent. Please understand that saying no does not remove your information from the RHIO system. We also will still have access to your information in a non-electronic format.

Canandaigua Medical Group introduces new appointment confirmation and outreach system for our patients.

AppointmentWe are pleased to announce a new proactive patient appointment confirmation and recall system that will help us better manage your care and help organize your busy life.

The friendly appointment confirmation system will remind you of the date and time of an upcoming appointment. The interactive system allows you to confirm the appointment or to cancel or reschedule the appointment if you need. Everything is done through your touch tone phone and there are even safety features allowing you to go back if you press the incorrect response.

The recall system is an innovative method of improving patient quality and safety. You can expect our system to contact you to:

  • Schedule Preventive Care Appointments
    Help manage your chronic conditions by reminding you that you are due for a follow up appointment.

This system is discreet and will only ask that you contact us to schedule an appointment without going into the details of your condition.

This is just another way in which we remain committed to providing quality patient care for all ages.

Welcome to Medicare

Look for your "Welcome to Medicare" letter in the mail. If you have received this letter, please call your doctor to schedule an appointment for your physical. You must take advantage of this offer within 12 months of being eligible for medicare, or it will no longer be available. You become eligible on the first day of the month you turn 65 years of age.

Medical Records

The Canandaigua Medical Group utilizes the services of Smart Document Solutions to copy medical records. This is a national service that is used widely throughout our region.

If you wish to have a copy of your medical records all requests will be processed and billed by Smart Document Solutions.

PATIENT SATISFACTION SURVEY

In an effort to improve the delivery of our healthcare services, in June of 1999 we began a biweekly survey of patients visiting our facility. The survey covers ten key patient satisfaction issues regarding the appointment, reception, and examination process. The results of this survey provides us with vital patient input enabling us to enhance the quality of our patient encounters.
