School, Camp, Sports Physicals

A Message Regarding Physicals........

The Pediatric staff of the Canandaigua Medical Group wish to encourage you to schedule physical examinations for your children for school, sports, camp and other requirements here at the office.

Don't forget to bring your Forms!!

The office physical examination is comprehensive and thorough. We use this opportunity to elicit and discuss any of your child's health concerns as well as performing the examination and reviewing any findings with a

parent. We also review lab and immunization requirements and update your child as needed. Most insurance's cover one physical examination yearly. In comparison, the examination your child receives at school is brief and there is no opportunity for input from the parent/guardian.

We encourage you to schedule your child's physical examination with one of our pediatric health care providers at your earliest convenience.

  • We can accommodate requests for physical's within 1 to 3 weeks at this time.
  • A parent or guardian is required to sign consent for all immunizations.

New York State requires a physical examination prior to entrance into kindergarten, then in 2nd, 4th, 7th, and 10th grades.

If your child participates in sports or goes to camp, yearly physicals are required.

Our pediatricians recommend yearly physical examinations for all patients so that we may be familiar with your child and we can keep up to date records.

A Message to Kindergarten Parents....

As September approaches, Before your child enters kindergarten he/she will need to be seen for a kindergarten physical. The Canandaigua Medical Group would like to take this opportunity to explain what is involved with a kindergarten physical. This may help you, as a parent, and your child, prepare for the appointment ahead.

New York State Law requires that a parent or legal guardian sign for a child (under the age of 18 years) to receive immunizations. Therefore, we do ask that a parent or legal guardian bring the child to the appointment. It would be helpful if you brought your child's immunization booklet with you to the appointment. You can read about each immunization that your child will receive here. You will need to be familiar with the information and be prepared to sign for them at the time of the physical.

The appointment will take approximately one hour. Due to the length of the appointment and the need for your child's undivided attention, we ask that you try not to bring siblings to this appointment.
